The London District Intercollaborative Model of Care (LDIM) is an innovative initiative that brings together optometrists (ODs) and ophthalmologists (OMDs) to provide comprehensive, collaborative eye care in the London community and beyond.
Our mission is to enhance patient access to specialized eye care services, reduce wait times, and optimize healthcare resources through a streamlined referral process.

Our Collaborative Approach
LDIM was developed to address the increasing demand for specialized eye care services in SW Ontario.
Access to (hospital-based) eye care is in crisis. LDIM’s innovative model aims to leverage the skills of both Optometrists and Ophthalmologists to manage patient care more effectively.
Through a centralized referral system, general practitioners (GPs), ODs, and other medical professionals can refer patients with specific eye concerns directly to an OD within the LDIM network.
Glaucoma Care
One of LDIM’s primary focuses has been on enhancing the management of glaucoma, a leading cause of vision loss that requires timely diagnosis and treatment.
Through our collaborative model, we have aimed to significantly reduce wait times for ophthalmology consultations and surgical interventions. Optometrists are trained to perform comprehensive eye examinations as well as treat glaucoma effectively. Advanced glaucoma or patients in need of a surgical solution to glaucoma can be referred to the Ophthalmologists on our team for timely intervention.
This seamless integration helps patients receive timely and appropriate care, reducing the risk of vision loss and improving overall outcomes.


Pediatric Eye Care
Children’s vision is critical to their development, and early detection of conditions like amblyopia, strabismus, or other eye-related complications from certain systemic conditions is essential for preventing long-term vision problems.
Our collaborative model allows for early detection and intervention by optometrists, with seamless referrals to pediatric ophthalmologists for specialized treatment available when necessary.
Our integrated approach helps children receive comprehensive, family-centred care that can support their visual health and development.

When you refer a patient through LDIM, they are first assessed by an OD within our network. If specialized care or surgical intervention is required, the patient is seamlessly referred to an ophthalmologist. When you are referred to LDIM, you will first see an optometrist who will assess your eye health and provide initial care. If your condition requires specialized treatment or surgery, your optometrist will refer you to an ophthalmologist within our network.
