The London District Intercollaborative Model of Care (LDIM) involves collaborating with ophthalmology and optometry to help enhance patient care.

Our initiative addresses the growing demand for eye care in London and surrounding areas by optimizing our healthcare resources. Partnering with the Ivey Eye Institute, we aim to reduce wait times and improve access to specialized care such as glaucoma management and pediatrics.

About LDIM

LDIM integrates optometrists (ODs) and ophthalmologists (OMDs) in a collaborative model that helps each professional work to its fullest scope efficiently and effectively. 

Patients can receive timely care from ODs and OMDs using a centralized referral system as needed in an effort to significantly reduce wait times.

Glaucoma

Glaucoma management is a cornerstone of our collaborative approach, demonstrating the partnership between optometrists and ophthalmologists.

The LDIM model maximizes the roles of optometrists in glaucoma care, enabling MDs to focus on complex and surgical cases. The priority is to fully utilize each provider’s skills and knowledge, improving patient access and outcomes by streamlining care and ensuring efficient management for all patients.

Pediatrics

LDIM is committed to providing integrated eye care services that help with the early detection, timely intervention, and management of pediatric eye conditions.

When pediatric eye conditions are identified, our collaborative model creates a seamless transition between optometrists and ophthalmologists, utilizing each profession effectively and efficiently to optimize patient access and care.

We offer a centralized fax number to streamline the referral process, allowing easy access to the appropriate specialists. A centralized system helps ensure patients are directed to the right provider quickly, reducing wait times and improving access to care.

Given our population’s changing demographics and trends in specialized eye care, a collaborative approach between Ophthalmologists and Optometrists ensures access to care for all of our patients.
